This year, Project GROW focused on two key projects: a Multicultural Community Health Literacy Program; and an Intellectual Disability Inclusion Program for general practice.  

Central and Eastern Sydney PHN commissioned CASS Care to deliver a Multicultural Health Literacy Program in Korean, Vietnamese, Chinese, and Indonesian to people with intellectual disability, their families and carers. The program reached 129 participants through six workshops, with 88.5% reporting improved intellectual disability health literacy. Resources were also distributed at 16 community events and an intellectual disability Annual Health Assessment social media campaign gained 23,656 views. 

The Intellectual Disability Inclusion Program was delivered by Central and Eastern Sydney PHN to 21 general practices across the region. Each practice participated in two education sessions on intellectual disability health care and completed two quality improvement activities. The quality improvement activities led to the identification of approximately 170 new patients with intellectual disability in GP practice software and the recall of ~260 patients for annual health assessments.  

The program demonstrated a significant increase in confidence among GPs and Practice Nurses in providing health assessments for people with intellectual disability. Feedback also highlighted the program’s positive impact on their practice and overall improvement to patient care.
