HealthPathways South East Sydney
- 621 live pathways
- 74 pathways localised and published since June 2023
- Acknowledged for developing quality pathways, with 18 pathways recently recommended and utilised as base pathways for other regions nationally
- Aligned with new statewide approach to improve review process, through safety checks and clinical relevance, in place of three-year review cycles for all pages
- Collaborated with SESLHD Strategy, Innovation and Improvement Project Teams to roll-out the NSW Health Engage Outpatients eReferral system
- Implemented NSW Health State-wide Referral Criteria across all relevant pathways for, Gastroenterology, Ophthalmology, ENT and Orthopaedics
- Undertook development of lead region pathways for aged care, HIV, ADHD in adults and menopause.

Primary Health Networks – Aged Care and Dementia Clinical Referral Pathways
In 2023-24 Central and Eastern Sydney PHN embarked on review, development, and enhancement of its aged care and dementia clinical referral pathways relevant to the health needs of the Central and Eastern Sydney region. These pathways are used by our clinicians during consultation with patients, to support assessment and referral to local services and supports.
Where existing aged care HealthPathways were already in place, these pathways have been reviewed and updated to improve the pathways and ensure currency, accuracy, and consistency with current best medical best practice and linkages to local services.
In the last 12 months, the Sydney and South Eastern Sydney HealthPathways teams have reviewed developed and enhanced in excess of 40 aged care and dementia pathways across three main areas:
- Cognitive impairment and dementia
- Maintaining healthy ageing
- Residential Aged Care.
Review of pathways across these areas has enabled the development of 15 new pathways.
