Integrated Team Care

Socio-economic target 1 of the National Agreement on Closing The Gap aims to ensure that Aboriginal and/or Torres Strait Islander people enjoy long and healthy lives.

The Integrated Team Care (ITC) program is a key element of this strategy which has an overarching objective to close the gap in life expectancy by 2031.   It is a dedicated service designed to provide one-on-one support to help eligible clients self-manage chronic disease conditions and access the primary health supports they may need.

Sydney Local Health District, South Eastern Sydney Local Health District and Sydney Children’s Hospital Network Aboriginal chronic care teams continue to provide this service.

ITC provides supports Aboriginal and/or Torres Strait Islander people with chronic diseases by:  

  • Assistance from clinical care coordinators and/or Aboriginal Outreach workers to help clients navigate supports and services 
  • Assisting with funding support for supplementary services for chronic disease needs including specialist and allied health appointments and specific medical aids/appliances   
  • Assistance with organsing and/or funding transport to medical appointments.  

Deadly Choices Program

Deadly Choices is a health promotion initiative of the Institute for Urban Indigenous Health. 

In Aboriginal slang, if something is ‘deadly’ it’s great, cool, or awesome. As such, Deadly Choices aims to empower Aboriginal and Torres Strait Islander people to make healthy choices for themselves and their families, to be role models and leaders within their communities and to complete their annual Aboriginal and Torres Strait Islander health check (MBS 715) to prevent and better manage chronic disease. 

The Sharks Deadly Choices program combines an 8-week schools and community healthy lifestyles program with community outreach delivered in collaboration with local GP practices. 

The 8-week program is delivered in a flexible manner to suit school environments and timetables, and addresses healthy lifestyle topics such as: 

  • Leadership 
  • Chronic disease 
  • Tobacco/Vaping cessation 
  • Nutrition 
  • Physical activity 
  • Harmful substances 
  • Healthy relationships 
  • Access and health checks 

The Cronulla Sharks have continued to deliver this program in the Sutherland Shire region and more broadly in other areas of the Central and Eastern Sydney region including La Perouse. 

ITC program initiatives are currently funded to June 2025 under the Australian Government’s Indigenous Australians’ Health Program.
